JOB SUMMARY
The Diabetes Care & Education Specialist 1 works with people at risk and with diabetes to achieve behavior change goals which lead to better clinical outcomes and improved health status.
ESSENTIAL FUNCTIONS OF THE ROLE
Plans, directs, and provides individualized self-management education, training and support to patients and families in a culturally-competent manner across the lifespan.
Develops and conducts educational programs and in-service training programs.
Serves as a resource to staff and community as a diabetes care and education specialist.
Optimize care delivery, patient and provider experience, quality, safety, and costs.
Applies current principles of teaching and learning and/or behavior change to facilitate self-management skills.
Tailors interventions to individual self-management needs.
Communicates information to patients and healthcare team in a timely manner. Serves as a role model, resource, and mentor to other team members.
Participates in quality improvement activities to support process improvement and clinic/facility/system goals.
Participates in professional development activities to maintain and improve required qualifications; obtain 15 diabetes-related Continuing Education hours annually.
KEY SUCCESS FACTORS
Advanced understanding of pre-diabetes and diabetes care modalities, therapies, terminology and equipment.
Demonstrates familiarity with pre-diabetes and diabetes pathophysiology, epidemiology, and evidence-based clinical practice.
Demonstrates familiarity with ADA Education Recognition requirements or National Standards for Diabetes Self-Management Education and Support, if in an ADA Recognized program.
Must be able to communicate thoughts clearly; both verbally and in writing.
Ability to effectively use a variety of teaching methods.
Knowledge of the geographic area, the community and its resources.
Cultural competency related to health practices.
Committed, dependable, adaptable, creative, resourceful and has strong desire to help the community.
Able to interact positively with patients as needed.
General computer skills, including but not limited to: using required software applications, data entry, information security, electronic medical documentation, hand held scanning and email.

QUALIFICATIONS
- EDUCATION - Bachelor's
- EXPERIENCE - Less than 1 Year of Experience
- CERTIFICATION/LICENSE/REGISTRATION - Basic Life Support (BLS): BLS or BLS within 30 days of hire or transfer.
Registered Dietitians (RD), Registered Nurse (RN), Pharmacist (RPH): Must have one of the following: -- RD - Registered Dietician -- RN - Registered Nurse -- RPH - Pharmacist.
