Job DescriptionI. General SummaryThe Social Work I position will be based at the THRIVE Clinical Program at UMMS Midtown Campus and coordinate with other IHV practices. This individual will provide direct, on-site clinical social work services to patients at the adult outpatient/ambulatory HIV clinic. This individual will provide behavioral health interventions and assist patients with all psychosocial needs relating to their HIV care. Under general supervision, provides therapeutic intervention and social work services to patients and their families to enhance comprehensive, integrated and uninterrupted care in the hospital and to have continuity of care in the community.
